Phase 3N = 436NCT00769704

Population
Patients with injectable melanoma that was not surgically resectable, stage IIIB to IV (the final-analysis abstract gives stage IIIB to IVM1c).
Design
Randomised, open-label phase 3 trial, 2:1, intralesional talimogene laherparepvec versus subcutaneous GM-CSF. Primary endpoint durable response rate (objective response lasting continuously 6 months or more) per independent assessment.
Primary endpoint
DRR
Arms
  • Talimogene laherparepvec (experimental, n = 295): Intralesional talimogene laherparepvec
  • GM-CSF (control, n = 141): Subcutaneous GM-CSF

Per the 2019 final-analysis paper, this primary OS analysis followed 290 deaths at a median follow-up of 44 months. The FDA Summary Basis for Regulatory Action reports that reviewers were concerned about informative censoring and, from additional survival data requested from the applicant, a revised P value of 0.116 (median OS 22.9 vs 19.0 months).

Unadjusted odds ratio 16.6 (95% CI 4.0 to 69.2). The paper states that all final analyses are descriptive and that responses were assessed by the investigators; median follow-up is that of the final overall survival analysis.
