NWTS-3
Third National Wilms' Tumor Study (NWTS-3)
N = 1,439
- Population
- Children with Wilms tumour in stages I to IV, favourable or unfavourable histology
- Design
- Cooperative-group study that sought to reduce treatment for low-risk patients and find better chemotherapy for those at high risk of relapse; 1,439 eligible patients, randomised by stage (I to IV) and histology (favourable or unfavourable). Randomised regimens named in the abstract: stage I favourable histology, actinomycin D plus vincristine for 10 weeks versus 6 months; stages II and III favourable histology, actinomycin D plus vincristine with or without doxorubicin for 15 months (stage II also 0 or 2000 cGy and stage III 1000 or 2000 cGy of postoperative radiotherapy); high-risk patients (any stage IV, all unfavourable histology), postoperative radiotherapy and actinomycin D plus vincristine plus doxorubicin with or without cyclophosphamide. The abstract gives stratum-level 4-year survival, not arm-level values.
- Arms
- 10 weeks of actinomycin D plus vincristine (experimental): Actinomycin D plus vincristine for 10 weeks (stage I favourable histology)
- 6 months of actinomycin D plus vincristine (control): Actinomycin D plus vincristine for 6 months (stage I favourable histology)
- Actinomycin D plus vincristine, 15 months (control): Actinomycin D plus vincristine for 15 months (stages II and III favourable histology)
- Actinomycin D, vincristine plus doxorubicin, 15 months (experimental): Actinomycin D plus vincristine plus doxorubicin for 15 months (stages II and III favourable histology)
- High risk: without cyclophosphamide (control): Postoperative radiotherapy with actinomycin D, vincristine and doxorubicin (high-risk patients)
- High risk: with cyclophosphamide (experimental): Postoperative radiotherapy with actinomycin D, vincristine, doxorubicin and cyclophosphamide (high-risk patients)
