International Germ Cell Consensus Classification: a prognostic factor-based staging system for metastatic germ cell cancers

Phase observational

Population
5,202 patients with non-seminomatous germ cell tumours and 660 with seminoma, treated with cisplatin-containing chemotherapy (poor-prognosis group is non-seminoma only)
Design
Pooled analysis of clinical data from collaborative groups in 10 countries on patients with metastatic germ cell tumours treated with cisplatin-containing chemotherapy; multivariate prognostic-factor analysis, with validation on an independent data set. Arms here are the three prognostic groups, not treatment arms; roles are schematic.
Arms
  • Good prognosis (control): Cisplatin-containing chemotherapy (60% of germ cell tumours)
  • Intermediate prognosis (experimental): Cisplatin-containing chemotherapy (26% of germ cell tumours)
  • Poor prognosis (experimental): Cisplatin-containing chemotherapy (14% of germ cell tumours, all non-seminoma)

Results reported for International Germ Cell Consensus Classification: a prognostic factor-based staging system for metastatic germ cell cancers
EndpointAnalysisIntermediate prognosis / Poor prognosisGood prognosisEffect
OSAll patients with data (non-seminoma and seminoma combined; the poor-prognosis group contains non-seminoma only)5-year survival rate by prognostic group, as reportedfinalmedian follow-up 5 yearsInternational 199779% at 5 y (75–83%)91% at 5 y (89–93%)Roles of the three groups are schematic: they are prognostic groups, not randomised arms, and must not be plotted as control versus experimental. The abstract reports 5-year survival and does not label it overall survival explicitly. The values pool non-seminoma and seminoma for good and intermediate prognosis; the 2021 non-seminoma update abstract quotes non-seminoma-only original values of 92% (good) and 80% (intermediate) with 48% for poor prognosis.
