EORTC 18991

Adjuvant pegylated interferon alfa-2b versus observation in resected stage III melanoma

Phase 3N = 1,256NCT00006249

Population
Patients with resected stage III melanoma
Design
Randomised phase 3 trial. Pegylated interferon alfa-2b 6 mcg/kg per week for 8 weeks (induction), then 3 mcg/kg per week (maintenance) for an intended 5 years, versus observation. Stratified by microscopic or macroscopic nodal involvement, number of positive nodes, ulceration, tumour thickness, sex and centre. Primary endpoint recurrence-free survival; analyses by intention to treat.
Primary endpoint
RFS
Arms
  • Pegylated interferon alfa-2b (experimental, n = 627): 6 mcg/kg per week for 8 weeks, then 3 mcg/kg per week for an intended 5 years
  • Observation (control, n = 629): Observation after surgery

The abstract states there was no difference in overall survival (no value given). Grade 3 adverse events 40% vs 10%; grade 4 5% vs 2%; discontinuation because of toxicity 31% of the interferon group.

No difference in overall survival (P=0.57; no hazard ratio in the abstract). In stage III-N1 ulcerated melanoma (a subgroup analysis; the abstract does not say whether it was prespecified), HR for RFS 0.72 (99% CI 0.46 to 1.13), for DMFS 0.65 (99% CI 0.41 to 1.04), for OS 0.59 (99% CI 0.35 to 0.97; P=0.006). Treatment discontinued for toxicity in 37%.
