COSMIC-311

A Phase 3, Randomized, Double-Blind, Placebo-Controlled Study of Cabozantinib (XL184) in Subjects With Radioiodine-Refractory Differentiated Thyroid Cancer Who Have Progressed After Prior Vascular Endothelial Growth Factor Receptor (VEGFR) -Targeted Therapy

Phase 3N = 187NCT03690388

Population
Patients aged 16 years or older with radioiodine-refractory differentiated thyroid cancer (papillary or follicular and variants), ECOG performance status 0 or 1, who had received lenvatinib or sorafenib and progressed during or after up to two VEGFR tyrosine kinase inhibitors
Design
Global, randomised (2:1), double-blind, placebo-controlled phase 3 trial of cabozantinib 60 mg daily versus matching placebo, stratified by previous lenvatinib and age. Co-primary end points: confirmed objective response rate per RECIST 1.1 in the first 100 randomised patients (OITT population) and progression-free survival in all patients (ITT), both by blinded independent radiology committee. Placebo patients could cross over to open-label cabozantinib at confirmed progression. The primary report presents the ORR analysis and a concurrent preplanned interim PFS analysis.
