CheckMate 649
Efficacy Study of Nivolumab Plus Ipilimumab or Nivolumab Plus Chemotherapy Against Chemotherapy in Stomach Cancer or Stomach/Esophagus Junction Cancer
Phase 3N = 1,581NCT02872116
- Population
- Previously untreated, unresectable, non-HER2-positive gastric, gastro-oesophageal junction or oesophageal adenocarcinoma, regardless of PD-L1 expression (about 70% gastric by primary tumour location in the 3-year report).
- Design
- Multicentre open-label randomised phase 3 trial in 175 hospitals and cancer centres in 29 countries. Patients were randomly assigned 1:1:1 while all three groups were open (nivolumab plus chemotherapy, nivolumab plus ipilimumab, or chemotherapy alone); this record covers the two arms compared in the primary report, 1581 patients concurrently randomised to nivolumab plus chemotherapy or chemotherapy alone. Chemotherapy was capecitabine plus oxaliplatin every 3 weeks or leucovorin, fluorouracil and oxaliplatin every 2 weeks. Primary endpoints for nivolumab plus chemotherapy versus chemotherapy: OS or PFS by blinded independent central review in patients with PD-L1 combined positive score (CPS) of 5 or more.

- OS, PFS
- Arms
- Chemotherapy alone (control, n = 792): Capecitabine plus oxaliplatin or leucovorin, fluorouracil and oxaliplatin
- Nivolumab plus chemotherapy (experimental, n = 789): Nivolumab (360 mg every 3 weeks or 240 mg every 2 weeks) with the same chemotherapy

- Nivolumab plus chemotherapy (experimental)
- Chemotherapy alone (control)
Median-only curves assume a constant hazard (exponential) and fit plateaus, as seen with some immunotherapies, poorly.
