Reducing uncertainties about the effects of chemoradiotherapy for cervical cancer: individual patient data meta-analysis (Chemoradiotherapy for Cervical Cancer Meta-Analysis Collaboration)

Phase meta-analysis

Population
Women with cervical cancer randomised in eligible trials of radiotherapy versus concomitant chemoradiotherapy
Design
Individual patient data meta-analysis of randomised trials comparing radiotherapy (with or without surgery) with concomitant chemoradiotherapy (with or without surgery). The JCO title refers to 18 randomised trials; the main comparison of chemoradiotherapy versus the same radiotherapy used 13 trials. Published accounts of how many trials were eligible and how many supplied data differ between the Cochrane abstract and the JCO full text, so only the figure of 13 trials is used. Searches in the Cochrane record were updated to October 2009.
Arms
  • Same radiotherapy alone (control): The radiotherapy given in each trial, without concomitant chemotherapy
  • Concomitant chemoradiotherapy (experimental): The same radiotherapy with concomitant chemotherapy (platinum-based or non-platinum-based)

OS13 trials comparing chemoradiotherapy with the same radiotherapyprimaryChemoradiotherapy 2008——HR 0.81P<.001The abstract reports a 6% improvement in 5-year survival with chemoradiotherapy; no confidence interval is given in the abstract. The Cochrane Database of Systematic Reviews record (PMID 20091664) republishes the same analysis and is not an independent second result.
OSTrials of platinum-based chemoradiotherapyexploratoryChemoradiotherapy 2008——HR 0.83P=0.017Subgroup of trials by chemotherapy type; the abstract states the analyses by trial and patient characteristics were prespecified. No confidence interval is given in the abstract. The subgroup table of the JCO full text lists slightly different values (HR 0.84, 95% CI 0.72 to 0.98), as read through a page-reading tool; the abstract values are shown as published.
OSTrials of non-platinum-based chemoradiotherapyexploratoryChemoradiotherapy 2008——HR 0.77P=0.009Subgroup of trials by chemotherapy type. No confidence interval is given in the abstract. The subgroup table of the JCO full text lists slightly different values (HR 0.76, 95% CI 0.62 to 0.94), as read through a page-reading tool; the abstract values are shown as published.
