Pretreatment cytogenetic abnormalities are predictive of induction success, cumulative incidence of relapse, and overall survival in adult patients with de novo acute myeloid leukemia: results from Cancer and Leukemia Group B (CALGB 8461)
Phase observationalN = 1,213
- Population
- Adults with de novo acute myeloid leukemia who received similar induction therapy.
- Design
- Prospective cohort analysis of 1213 adults with de novo AML who all received similar induction therapy; outcomes were complete remission rate, 5-year cumulative incidence of relapse and 5-year overall survival; median follow-up of surviving patients 8.3 years. Groups were defined from the outcomes of specific abnormalities and karyotype complexity, not assigned: arm roles are a schema requirement.
- Arms
- Favorable cytogenetic group (experimental): Favorable-risk cytogenetics at diagnosis (as defined in the paper)
- Intermediate cytogenetic group (control): Intermediate-risk cytogenetics at diagnosis (as defined in the paper; reference group in multivariate analyses)
- Adverse cytogenetic group (experimental): Adverse-risk cytogenetics at diagnosis (as defined in the paper)

| Endpoint | Analysis | Favorable cytogenetic group / Adverse cytogenetic group | Intermediate cytogenetic group | Effect |
|---|---|---|---|---|
| Complete remission rateAll 1213 adults with de novo AML, by cytogenetic risk group | primaryByrd 2002 | 88% | 67% | Cohort analysis; the abstract gives no confidence intervals. In multivariate analysis the probability of achieving CR in the adverse group was 4.0 and 11.9 times lower than in the intermediate and favorable groups. |
| Cumulative incidence of relapseAll 1213 adults with de novo AML, by cytogenetic risk group | primaryByrd 2002 | 54% at 5 y | 67% at 5 y | 5-year cumulative incidence of relapse as published. In multivariate analysis the probability of relapse in the adverse group was 3.0 and 4.4 times higher than in the intermediate and favorable groups. |
| OSAll 1213 adults with de novo AML, by cytogenetic risk group | primaryByrd 2002 | 55% at 5 y | 24% at 5 y | 5-year overall survival as published. In multivariate analysis the risk of death in the adverse group was 2.1 and 4.3 times higher than in the intermediate and favorable groups. |
