Chemotherapy of advanced Hodgkin's disease with MOPP, ABVD, or MOPP alternating with ABVD

N = 361

Population
Newly diagnosed advanced Hodgkin's disease, stages IIIA2, IIIB, and IVA or IVB; patients in first relapse after radiation therapy were also eligible. 361 eligible patients.
Design
Randomised, multicentre trial of three regimens of primary systemic therapy: MOPP alone for 6 to 8 cycles, MOPP alternating with ABVD for 12 cycles, or ABVD alone for 6 to 8 cycles. Patients were stratified by age, stage, previous radiation, histologic features and performance status. No additional radiation therapy was given. Patients without a complete response, or who relapsed, on MOPP alone or ABVD alone were switched to the opposite regimen.
Arms
  • MOPP (control, n = 123): MOPP (mechlorethamine, vincristine, procarbazine, prednisone) alone for 6 to 8 cycles
  • MOPP alternating with ABVD (experimental, n = 123): MOPP alternating with ABVD (doxorubicin, bleomycin, vinblastine, dacarbazine) for 12 cycles
  • ABVD (experimental, n = 115): ABVD alone for 6 to 8 cycles

CR rateEligible patients (361)finalCanellos 199283%67%P=0.006P is for the comparison of MOPP with the other two regimens, both of which contained doxorubicin. Across all 361 eligible patients the overall response rate was 93% and the complete response rate 77%.
FFSEligible patients (361)finalCanellos 199265% at 5 y50% at 5 yRates at five years. The abstract gives no p-value for failure-free survival; it states that age, stage (III versus IV) and regimen influenced failure-free survival significantly.
OSEligible patients (361)finalCanellos 199275% at 5 y66% at 5 yP=0.28Rates at five years. P is for the comparison of MOPP with the doxorubicin-containing regimens.
